Work Experience Forms

Need a work based training form to report your hours? Download the following CTC form.

REMEMBER we need a journey persons NAME, TQ number and SIGNATURE to report the hours to ITA. We will not accept your work based training forms without this information.

If you or your employer have any questions regarding this form please don't hesitate to contact the CTC office and we'll be happy to explain the process.

On occasion the CTC office will be contacted by employers looking to hire students. If you are interested in having the CTC Office contact you if an opportunity becomes available while you're attending our programs please contact us.

Lost ITA Certificate/Wallet Cards

If you lost your ITA registration cards you must fill out a ITA replacement form and submit it to the ITA with payment.

• Certificate/Wallet Card Replacement

The CTC office will have your registration number on file.

Obtaining your College Certificate or Welding Log Book

On completion of Grade 12 you must bring in your ORIGINAL graduation transcript to the ADMISSIONS DEPARTMENT at the College of New Caledonia.

Welding students should contact the Industry Training Authority (ITA) on completion of their work experience hours, at which point students must send their log books to ITA to obtain their Welder Level C status.

Changing Employers / Sponsors

When CTC students with ITA registration cards find an employer or change employers they must provide ITA with the name of their new employer and submit the appropriate ITA form.

Continuing your Apprenticeship

If you wish to continue your apprenticeship at the College of New Caledonia after graduation, you must submit a NEW CNC APPLICATION
